60 minute table massage- $100
90 minute table massage- $150
120 minute table massage- $200

NEW CLIENT FORMS
If you are a new client (or if your last visit was more than 1 year ago), please email me at massagefromanna@gmail.com to request a health intake form. You will need to bring the form with you to your first session.
You may also arrive a few minutes early to your first session and fill out the health form in person.
If you have any health conditions that might require a physician's permission (recent surgery, pregnancy, circulatory issues, recent stroke), before receiving massage, I also might also ask for this form to be completed before our session. If you are not sure, please ask when booking the appointment.
PAYMENT POLICIES
- Payment is due at time of service. I currently accept cash, check or credit card (via Square).
- I do not do insurance billing, however if your insurance allows for out of network massage providers, be sure to ask about a receipt so you can use it for reimbursement.
- If paying by check that comes back NSF, then the client is liable for the amount of the check plus any additional banking fees. NSF checks and bank fees must be resolved within 30 days.
- If using a gift certificate, the dollar amount purchased is the dollar amount available for use. If prices have changed since original purchase, the difference must be covered by client on day of service.

CANCELLATION POLICY
- A 24-hour advance cancellation/rescheduling notice is required ahead of time to avoid paying the full session fee.
- If a client repeatedly does not adhere to above rules, the therapeutic relationship may be terminated.
- A client that forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged/billed for their missed appointment.
- If client arrives late, the session may be shortened in order to accommodate others whose appointments follow. This is especially true now that Covid-19 requires stricter cleaning/PPE procedures before and after sessions.Depending upon how late the arrival, I will determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, client will be financially responsible to pay for for the full session. Please plan accordingly and be on time.
- If you feel sick, think you may have been exposed, or have any sort of fever, chills, ANYTHING that might indicate you are getting sick, please let me know IMMEDIATELY when you are feeling ill so your appointment can be rescheduled.
